Lewis started her career in Eubie in 1979. She next landed the role of Effie White in Dreamgirls, but Jennifer Holliday was cast in the role. Lewis portrayed Tupac Shakur’s mother in the film Poetic Justice. Lewis was cast as one of Whoopi Goldberg’s backup singers in the 1992 film Sister Act. Judge Rosetta Reide, the first major African American lesbian lead on television, played in 1995.

In 1996, Lewis played Theresa Randle’s mobile sex line manager in Girl 6. Later that year, she portrayed Whitney Houston’s character’s mother in The Preacher’s Wife. She has also appeared in The Mighty, The Temptations TV miniseries, and Blast from the Past. In 2000, in the action thriller film Cast Away, Lewis had a supporting role. In 2000, on the Lifetime Television medical drama Strong Medicine, she appeared as Lana Hawkins. In the Pixar Cars movies, she voiced Flo as well.

On April 22, 2008, Lewis took over as Motormouth Maybelle in Broadway’s Hairspray, replacing Darlene Love. Clint Eastwood cast Lewis in his fantasy film Hereafter in 2010. Lewis appeared on the short-lived NBC series The Playboy Club. She portrayed Terrence J’s overbearing mother in the box office smash Think Like a Guy. Lewis was cast as Ruby Johnson, the mother of Anthony Anderson’s character, in the ABC comedy series Black-ish in 2014. She was nominated for a Critics’ Choice Television Award for Best Guest Star in a Comedy Series in 2016 for her performance.
