After 20 years at the practice, Osteen returned to Houston to assist Joel, his brother, in taking over as pastor of Lakewood. Osteen felt compelled to use his surgical training to assist those without access to medical care after a 2005 medical mission trip.

In 2005, Osteen decided to devote his time and efforts to addressing the lack of basic surgical care for people in Sub-Saharan Africa. Osteen travels to rural areas of Zambia and Kenya with Christian aid organizations like Samaritan’s Purse and World Vision to deliver medical supplies, train new doctors, and provide surgical care to communities that would otherwise be without access to healthcare.

Osteen also travels to affected areas immediately following catastrophic events and serves on teams that respond to immediate disasters, such as the refugee crisis in Iraq. In 2016, Osteen established the M3 Conference, a medical conference dedicated to missionary work. The conference aims to connect medical professionals with organizations working to end poverty in developing nations.
